What is this service?

GP-Led Medicines Management Team from Bespoke Clinical Services: a complete medicines team: pharmacists do the medicines work, a GP makes the medical decisions. Every arrangement is built around your organisation — truly bespoke, never off-the-shelf.

A combined pharmacist and GP service for practices that need medicines work handled end to end. Pharmacists undertake agreed tasks within their competence; supplied GPs support medical decisions, pathology interpretation, complex cases and prescription authorisation where required.

Pharmacists typically cover repeat and acute requests, reauthorisations, medication reviews, discharge reconciliation and monitoring recalls. Anything outside their competence or agreed scope is escalated to the GP, who makes the medical decision and authorises prescriptions where needed.

Delivery follows the commissioning practice's agreed clinical governance, policies and procedures. Before work begins, we agree scope, system access, authorisation, escalation, incident reporting and handover. Each clinician retains professional accountability; BCS provides the agreed workforce management, training and quality-assurance support.
